Passport Renewals
Renewals for adults
You may apply for a renewal of your passport if your previous passport:
1. was issued when you were 16 years of age, or older;
2. was issued within the last 15 years;
3. is not damaged; and
4. is submitted upon renewal.
If you cannot meet the above conditions, please follow the instructions for first-time applicants.
Please note that remarks on www.travel.state.gov are intended for domestic passport applicants, not overseas applicants. The U.S. Consulate Guadalajara requires the same form for the passport.
• Required Documents:
1. The completed form DS-82;
2. Current government-issued photo identification;
3. Two identical 2 X 2 inches (5 x 5 cm) passport photos; and
4. Your current passport.
Note: By federal law, you must provide a Social Security number if you have one.
Renewals for minors under age 16 must have parental consent:
1. Both parents must personally appear with valid photo identification.
2. If only one parent is present at the time of application, it is necessary to bring:
a. Form DS-3053 (Statement of Consent: Issuance of a Passport to a Minor) signed by the absent parent before a U.S. Consular Officer or Public Notary, or
b. U.S. notarized written consent (in English) signed by the absent parent authorizing the applying parent to request a U.S. passport for the minor child, and
c. A copy of absent parent’s identification with a clear photograph and signature.
3. If only one parent has sole custody it is necessary to bring proof that the parent executing the application has sole custody of child may also be accepted (i.e., birth certificate listing the mother as the only named parent, court order granting sole legal custody to one parent, or death certificate of deceased parent).
• Required Documents (Renewals for minors under 16):
1. The completed DS-11 form;
2. The applicant's original birth certificate showing the parent(s')
name(s);
3. Proof of child's U.S. citizenship, such as, an original or certified copy of a U.S. birth certificate, Consular Report of Birth Abroad, or a Naturalization certificate;
4. Current photo identification of the child;
5. Current photo identification of both parents;
6. Two identical 2 X 2 inches (5 x 5 cm) passport photos; and
7. Current passport.
