Renewals for adults
You can apply for a renewal of your passport if your previous passport:
(1) was issued when you were 16 or older;
(2) was issued in the last 15 years;
(3) is not damaged;
(4) is submitted upon renewal.
If you cannot meet the above conditions, please follow the instructions for first-time applicants.
Adults renewing their passports should fill out this form (DS – 82)
Please note that remarks on www.travel.state.gov are intended for domestic passport applicants, not overseas applicants. The U.S. Consulate Guadalajara requires the same form for the passport.
Required Documents
Bring the complete form DS-82, current government-issued photo ID, and provide two 2 X 2 inches (5 x 5 cm) passport photos, and your current passport.
By federal law, applicants must provide a Social Security number if they have one.
Renewals for minors under 16 (Form DS-11): Both parents are required to appear at the time of the application for children under age 16. If one of the parents is unable to be present at the time of application, then a notarized statement of consent of the non-present parent must be submitted at the time of the passport application. Passport renewal applications for a minor whose parent is listed on the birth certificate and whose whereabouts are unknown will be considered on a case by case basis.
Required Documents
the applicant's original birth certificate that shows the parent(s') name(s);
proof of child's U.S. citizenship such as an original or certified copy of a U.S. birth certificate, Consular Report of Birth Abroad, or a Naturalization certificate;
current photo identification of the child and both parents.
